This is a cool and useful flash contact with departments drop-down menu. It's resizable and easy to customize. It also has the ability to display title, introduction text and contact text (html & css formatted so that it can support links, color and size changes, bold, italic etc.). It also has an auto-responder so that your users get notified when they use your form. You can customize the e-mails sent (which are also html formatted not plain text) and a ton of other settings in the .php file (help/instructions are provided).
